Kids Exhibition Registration

Welcome to the Asia Flag League Kids Exhibition Game!

This registration form and waiver must be completed by a parent or legal guardian for any child participating in this event. Please read carefully and complete all required fields.

By submitting this form, you are registering your child to participate and agreeing to the terms outlined below.

I understand that flag football is a physical sport involving running, movement, and potential contact with other players, which may result in injuries including but not limited to:

• Sprains, strains, or minor bruises
• Collisions with other players or ground impact
• Dehydration or heat-related issues
• Other unforeseen injuries during play

In consideration for allowing my child to participate, I hereby release and discharge Asia Flag League, its officers, coaches, referees, staff, sponsors, partners, and venue operators from any and all claims, demands, and causes of action arising from my child's participation in this event.

This release covers personal injury, illness, damage to personal property, and any related medical or other expenses that may arise.

I understand that this waiver is binding upon myself, my child, and our heirs, executors, and administrators.

I authorize Asia Flag League event staff to provide or arrange basic first aid or emergency medical treatment for my child if needed during the event.

I understand that any further medical care, treatment, or expenses beyond basic first aid are my sole responsibility.

I affirm that my child is in good health and physically fit to participate in this flag football activity.

I grant permission for Asia Flag League and its authorized partners to photograph, film, or record my child's participation during this event.

These materials may be used for promotional purposes, social media content, event highlights, marketing materials, and future Asia Flag League communications.

I waive any right to compensation or prior approval for such use.
